Wellness Home Behavioral Health Services can provide and assessment, supportive counseling, and linkage to more intensive levels of substance abuse treatment through one of our local community and / or hospital-based partnerships. Each substance abuse partner provides services though best practices and evidence-based medicine.
Primary care screening, monitoring and linkage to primary care services utilizing key health indicators. For example, preventative care and screening of Body Mass Index (BMI) weight assessment and counseling for nutrition and physical activity for controlling blood pressure.
The LGBTQ community of Chicago has experienced Trauma, often simply due to trying to exist within the community. As such, Trauma Informed Care has been incorporated into all programming.
For many boys and men of color, traumatic experiences may become an almost routine part of everyday existence. Besides violence, assault, and other traumatic events, African American and Latino males often experience more subtle and insidious forms of trauma. Their exposure to discrimination, racism, oppression and poverty is pervasive.
