Wellness Homes of Chicago accepts most major credit cards and is happy to provide payment options by telephone. For questions about your balance or to make a payment, please call our Customer Service Specialist at 773-296-2400. Payment is usually due the date of service unless other arrangements have been made in advance. Self-pay patients are responsible for the entire amount of the bill. Patients participating in health plans are responsible for any amounts for any amounts not covered by their insurance. Co-pays are collected at each visit.
Our Wellness Homes accepts most commercial insurances. However, not all locations accept the same insurance. Please make sure to inquire before making an appointment. If you have an HMO, please call your HMO to ensure our location is in network.

Individuals with HMO, you can only go to an appointment with your Primary Care Provider (PCP) or obtain a referral from your PCP in order to see a different doctor. If a patient has UHC, Cigna and Aetna, they will have to select Dr. Brownlee as their PCP.
